Yes. The requirement covers travellers entering and leaving, so the outbound journey needs its own submission. Travellers who file it once on the way in and assume the trip is covered discover otherwise at the departure counter — which is the worst possible moment for a form.
Which authority runs the border?

The Department of Home Affairs, through its immigration officers at every port of entry. It also sets the rules for children and issues the visas, so unlike countries that split these functions across agencies, South Africa concentrates them in one place.
Why are the blank pages such a big deal?

Because South Africa still uses physical stamps and needs somewhere to put them, and because the requirement is applied literally rather than sympathetically. The land crossings to Namibia, Botswana, Zimbabwe, Mozambique, Eswatini and Lesotho are busy and ordinary, and the traveller declaration requirement covers them alongside the airports. An overland exit is not something an airline system can see, though, which is why a flight booking is often the practical answer to a check-in question.
What happens if I overstay?

Can I extend a ninety-day stay?

Do the neighbouring countries share the ninety days?

No. South Africa, Namibia, Botswana and the others each grant their own period, and a trip to Victoria Falls does not pause a South African count or extend it. What it does do is generate a new set of stamps, which is another argument for having those two blank pages.
Is there a departure tax?

It is included in the ticket price rather than collected at the airport. The paperwork you need at departure is the traveller declaration for the outbound leg, and a passport whose entry stamp is legible and unexpired.
How thorough are the arrival checks?

Documentary rather than interrogative. Officers at OR Tambo and Cape Town check the passport condition, the pages, the stamp and the declaration. Questions about your plans are less common than in Europe or North America; the scrutiny is on what you are carrying rather than on what you say.
What should be ready before I fly?
